By taking biopsies from long COVID patients before and after exercising, scientists in the Netherlands constructed a startling picture of widespread abnormalities in muscle tissue that may explain this severe reaction to physical activity.
Among the most striking findings were clear signs that the cellular power plants, the mitochondria, are compromised and the tissue starved for energy.
"We saw this immediately and it's very profound," says Braeden Charlton, one of the study's authors at Vrije University in Amsterdam.
The tissue samples from long COVID patients also revealed severe muscle damage, a disturbed immune response, and a buildup of microclots.
"This is a very real disease," says Charlton. "We see this at basically every parameter that we measure."
I feel insane seeing stuff like this because this research already exists for Myalgic Encephalomyelitis, a post viral condition caused by multiple types of viral infections that a LOT of people with "Long Covid" meet the diagnostic criteria for.
This article mentions that ME/CFS is a "similar complex condition" but that's DEEPLY underreporting the similarities. The phrase "post exertional malaise" (now researchers are trying to replace it but this article uses that phrase) was INVENTED for ME/CFS. It's the only known condition, before "long covid", that causes these kinds of symptoms after exertion!
It's good to know for sure that it's the same mechanisms at play when the inciting viral infection is Covid and not, for instance, Epstein-Barr or RSV, but half the time it doesn't seem like researchers are making comparisons at all, just reinventing the wheel and acting like "long covid" is a totally new phenomenon with no previous point of comparison. There are literally drugs in human trials to try to treat the mitochondrial dysfunction in ME, this dysfunction is well-established and fairly well understood and I feel insane when ppl report on long covid without mentioning that there is already a named and studied condition that accounts for this subset of symptoms!!
SOME researchers are drawing comparisons but they're largely ME researchers who everyone else is largely cignoring because of the widespread perception that ME is a fake disease for lazy women.
That same perception btw is why "graded exercise therapy" (GET), or exercise gradually increasing in intensity, cwas for years the go-to treatment despite MOUNTAINS of evidence that it makes ME patients sicker. Some end up permanently bedbound and unable to even eat or drink without a feeding tube/IV because the damage is so bad! The GET recommendation was finally changed only in the past few years in the US and the UK, and many doctors hate that they're not allowed to recommend it anymore, because they insist despite the evidence that ME/CFS is psychological and ME patients are just "deconditioned" and too lazy to do anything about it.
Now the same kind of "treatment" is being recommended for long covid patients despite evidence showing exercise is having the same kinds of cellular effects as it does in ME patients. "Taking PEM into account" sounds gentler but I'm deeply concerned about the reinvention of GET for patients who meet all the criteria for an illness that's been shown definitively to become permanently worse with GET. This mitochondrial damage is progressive in ME, and there's no reason to believe patients who meet all the criteria of ME after Covid won't experience the same progression if they force themselves past their energy envelope in such a systematic way.
The additional finding that T cells — part of the immune system's arsenal — had infiltrated the muscles of long COVID patients also caught Iwasaki's attention, possibly indicating "an autoimmune response within the muscle cells."
This autoimmune response is well-researched for ME. Every time you overexert, you're injuring your cells more and more. It's deeply worrying that rebranded GET is being recommended for long covid patients who meet the criteria for ME when research like this study keeps showing it's the same phenomenon.

i see this post going around a lot and i just want to like. add my two cents as a trans adult who transitioned as a child medically years ago. I agree with the above points, but need to just mention that being a person who medically transitioned as a child and was supported by their parents doesnt mean that transition was easy and free of suffering either. Transition doesnt need to have pain in it, but transitioning as a child is a very painful experience. I knew peers who were kicked out of their schools, forced to move across the country, had the support of one parent but not the other and had to deal with a divorce as a result to even be able to legally access care at all, and nearly every one of my medical providers have told me they recieve daily threats for doing what they do. Trans children are one of the biggest targets of harassment and abuse in this community. Saying that trans kids have it easy doesnt just show a disturbing view of transness having a requirement of suffering- it shows a complete denial of the suffering of trans children. I personally had it much easier than many of my friends, but having that experience makes me feirce and insistant that people know the shit that kids in our community have to go through on a daily basis.

Wait, listening to the same song on repeat is stimming?
I do not mean a normal amount of listening to the same song on repeat I mean that in October and November 2022 the only thing I listened to was Type O Negative's "Black No. 1" and I listened to it for 18 hours a day. This is, apparently, a kind of stimming.

To be clear I do not know what the normal amount is. When Large Bastard and I started dating I had a tape in my car with Tom Petty's "Dancing at the Zombie Zoo" on it that I knew exactly when to flip the other side (halfway through "Yer so Bad") so that I could flip it again and listen to Zombie Zoo again and I did that enough that Large Bastard still can't listen to Tom Petty and also it wore through the tape. So whatever normal is it is probably less than that.
Nope. And one of the things that made my doctors wave off the possibility of a diagnosis was "well, you don't stim or have trouble socializing or have sensory issues so you're probably just depressed and anxious."
No I just, you know, reexamined every interaction for hours after the fact and was convinced that I was evil or deeply broken for being unable to emotionally connect with people around me and had headphones hidden under my hair all the time to listen to songs on repeat so that I could pay attention to things and not get distracted by the way the classroom lights were flickering at exactly the wrong frequency.

I think the thing that annoys me most about AI on a personal, day to day, level is what it has done to grammar checkers. If you've never done a lot of editing, or used to 5+ years ago but haven't really in the last couple years, I can't even begin to describe how fucking BAD this shit has gotten. And as an author it is EXHAUSTING.
I just want to catch spelling errors and accidental double spaces and repeated phrases and whenever I use the wrong too/to or affect/effect and shit. But no. They've shoved AI up the ass of every grammar checking software out there and now they all fucking suck and make the most random, obnoxious, nonsensical suggestions.
And yeah, I can ignore all the times it's trying to get me to cut out any semblance of my own voice, or shove things into the wrong tense, or make the most random suggestions on comma usage. But if it's getting all that WRONG, what is it just straight up missing that I SHOULD be correcting? What real spelling and grammar errors are still lurking in there?

I get why people keep saying this (and other versions of it like "Use Adobe alternatives" and "Use Google product alternatives."). But here's the problem: I do not create in isolation. Even my own 100% personal projects are getting sent to other people whether it's editors or printers or beta readers and unless every single person in that train is using the same products, things can get wonky.
Libre Office and Word handle formatting differently on the back end, which can completely break documents if you move them back and forth between the two. So if I write in Libre Office but my beta readers are still using Word, when I send them a manuscript for review there's a good chance things won't look right and my beta reader will not actually be reviewing what I sent them.
Industry standards are industry standards FOR A REASON. Having everyone on the same workflow can be crucial to getting things done effectively and correctly without creating a lot of extra work. And those things are not going to change overnight, as much as we might want them to.

Yeah, Word, let me just leave this whole chunk of dialogue without the closing quotation marks. That's the thing to do. How dare I have two punctuation marks in a row. It's not like that's how closing quotation marks fucking work.

And you know, for young writers, this has got to be so detrimental just from the perspective of opening your document and seeing a million corrections that, frankly, don't need to be there. If you're a young writer you're likely not going to have the background knowledge to know what is and isn't a good suggestion, you're just going to see a document that makes it look like you made every mistake possible so clearly you must be a terrible, stupid writer and should just give up.
That is The Point. I have to address this with my students every semester. A lot of these “AI grammar check” programs will always, always identify things that “need fixing” regardless of whether there are actually any errors in the document.
They are, I am certain, intentionally programmed that way because the user is supposed to see “errors” coming up every time they write anything and conclude that they Need the AI. That they CAN’T write without the AI. “Look how many things it fixed — I made so many mistakes — I never would have caught all this without the AI.” So they keep using it, because otherwise all their documents will be full of “errors” that they or their human proofreaders all “missed”.
I do a live “fuck Grammarly” demo near the beginning of each semester, where I give it a paragraph from a published work and point out all the mistakes it claims are there, very few if any of which are genuine mistakes of any kind. It also displays that “grammarly score”, which tends to be aggressively low, especially if the text is at all interesting or original — “let’s see what it thinks of some of the stuff you were assigned in high school English as examples of Great Works… ooh, Edgar Allan Poe gets a C, he should learn how to write.”
And, of course, any time you tell it to rewrite something, it can’t come back with “this is already good actually, you don’t need a subscription to Grammarly at all” — if given a text that is already perfectly fine, it will just randomly swap out various words for synonyms. Because it always has to Do Something. If the program makes no changes, you might decide you don’t need it. But if you’re not a confident writer, if Grammarly tells you this synonym is “better”, you’re likely to believe it.
So yeah, the people who program these things absolutely want everyone who uses them to decide they’re a Bad Writer and give up. The “AI” is supposed to be hypercritical and find problems everywhere. Because people who KNOW that they can write decent prose on their own won’t use the software.
