Did I just employ the "Treat Them Like You are A Kindergarten Teacher Again" method with my insurance company today? I surely did. Did it work? Probably better than intended because I made an actual doctor feel contrite.
So, my insurance has been trying to not cover my SNRI because it is new on the market and no generic available yet, so pricey.
I apply for a refill and the request gets locked for review. Again. For the 3rd time.
This time I call and immediately ask to speak to the actual doctor making these clinical decisions. Very politely. Must be a slow day because they allow it.
ME: [Teacher voice] I'm calling in regards to the SNRI you have placed a lock on. Why was this decision made?
DOC: Well, there are dozens of other medications on the market in that tier, and far cheaper for you and [insurer]. We have sent a request to your doctor to consider alternatives.
ME: I am aware of that. So, can you do me a HUGE favor and look up my prescription history really quickly and tell me how many SSRIs and SNRIs were only filled once in 2022 for me, showing they were poorly tolerated?
DOC: It looks like eight.
ME: Great job! Now, can you please look at my genetic test for psychiatric drug tolerance and tell me how many medications are listed in the safe category?
DOC: Two.
ME: Awesome! Now, can you tell me what type that other drug is that I'm not taking?
DOC: Yeah, totally, it's an MAOI.
ME: That's correct, you're really knowledgeable! Should I be taking something as dangerous as an MAOI with my other medications, or even just in general?
DOC: It's contraindicated for sure.
ME: It is! So true! So, last question since you've been incredibly smart and helpful. Is it less expensive for [insurer] to pay out for the medication knowing they already get a huge manufacturer discount anyway, or is it more expensive for them to pay for me to need potentially long-term inpatient psychiatric care?
DOC: I'll clear the code, ma'am and flag it as medically necessary. I'm sorry about this.
ME: I appreciate you SO MUCH. You have a great day now.

When you said in your Katherine Hepburn post that you wonder how many transmasculine people kept their identities close to their chest to be the wife or mother that they were supposed to be, I always think of my neighbor.
I live in an extremely Mormon community in rural Northern Utah. I came out as a trans man as a teenager, about 10 years ago. My neighbor— someone who I had always known as a traditional Mormon woman in her early 40s, a devout housewife, a mother of several children, a valued community member from a very important family in our area (her brother is literally the mayor of our town), and at most SLIGHTLY more reclusive and quiet than most of the bigshots in our community— quietly told me on their front porch one evening that they have always seen themself as a man.
They wistfully told me about their college years, where they were involved in the lesbian community, before in their early 20s realizing that it was “more” than that. About how they only moved back home and got married after their college degree because the thought of being a disappointment to their family, and being disowned, felt impossible to cope with. So they got married to a man that they admit they don’t feel any ounce of attraction towards, and had several kids, and they’re not quite the pride to their parents as their siblings (due to being slightly more reclusive, not really having friends to speak of that aren’t just church ladies that they work with, and, admittedly, always seeming a bit depressed to me). They said that they were proud of me for doing what I needed to do, though, and they were happy that my family at least wasn’t disowning me.
Nobody else in our community knows. Not their family, not their husband, not their kids, not the people they do church outreach with.
And whenever I read stories of forgotten trans men, I always think of them. They’re still alive! They’re still here! And nobody but me will probably ever know! And if I hadn’t come out in this tiny little community, I’d never know either!
Idk. Sorry for the vent/rant. Thank you for listening. It crushes me to be the only one who knows them as a man, sometimes.

I think that part of what irks me about AI being used even for "small" stuff like meme graphics (aside from all the larger problems obviously) is like.... the time spent on it was part of the humor. Someone really took time out of their day to make me look at something silly they took time out of their day to make. Now it's like someone took time out of my day to make me look at something no one even made. Stop wasting my time if you aren't wasting yours too.
