“Name?” the triage nurse asks.

Joel answers automatically. “Ellie Williams. Fourteen.”

Ellie doesn’t look up.

Around them, shift change is happening. Voices overlapping, pens clicking, sounds coming together to create the low hum of a place that never sleeps. Ellie stands very still behind Joel’s broad back, eyes unfocused, like she’s watching something far away that no one else can see. 

The nurse at the window glances at the arm. “Chemical?”

Joel nods. His jaw tightens. “Drain cleaner. I think. I didn’t—” 

He stops. Swallows. “I didn’t see.”

They’re already being waved back.

The cartoon animals covering the colorful walls make the sinister situation almost seem comical. A gray-haired nurse with kind eyes and a nose ring checks Ellie’s vital signs as she sits perched at the edge of a bed; her blood pressure, pulse, temperature. Everything normal for the circumstances.

Joel doesn’t mind seeing a doctor for his own health, although he avoids going for anything less than an open fracture, but it bothers him more to see someone else being poked with needles. The nurse cracks a joke to calm him down, and he knows it will be fine. But the fear sits deeper than that. The last time he sat with a little girl in the hospital, he lost her. 

The curtain is pulled aside and someone steps in. 

“Good morning, I’m Doctor Jack Abbot.” The man is in his late forties maybe. Dark hair going gray at the temples. There’s an ease to him, but also something coiled underneath it. He looks tired in a way Joel recognizes. Bone-deep, earned.

He pulls a pair of blue plastic gloves on as he sits down on a rolling stool so he’s level with Ellie. “How are you feeling, sweetheart? Any pain at the moment?”

No response.

Jack’s eyes flick briefly up to Joel, who is shifting at the foot of the stretcher. A question there, unspoken.

“She’s been like this all morning,” Joel says. “She’s usually… not.”

Jack hums. “Yeah. Teenagers aren’t known for their silence, are they?” A beat. “The day attending is gonna take over in a minute, but let’s have a look first.”

Ellie’s fingers twitch as the doctor assesses the damage. The first reaction Joel’s seen from her in hours.

Jack leaves, and the nurse cleans the burn carefully. It’s worse than Joel thought; angry red and white patches along her forearm, skin already blistering. Ellie doesn’t flinch. Doesn’t cry. Doesn’t react at all.

Joel has to sit down. He makes sure to leave Ellie space as he plumps down next to her.

By the time Doctor Robinavitch walks in, the sun is fully up. He fills the room in a way the sun can’t inside the windowless space; broad shoulders, calm voice, hands steady as he reviews the chart.

“Hey, Ellie,” Robby says gently. “I’m Doctor Robby. I’m gonna be hanging out with you for a bit.”

Nothing.

Robby doesn’t push. He talks anyway, narrating what he’s doing, explaining the treatment plan, giving Ellie control where he can. Joel watches him closely. This is a man who knows how to talk to people in pain.

Eventually, after the dressing is secured and the room goes quiet again, Robby asks softly, “Ellie, can you tell me what happened?”

Silence.

Joel feels something old and sharp crawl up his spine. He knows this quiet. The danger of it.

“I burnt myself.” They wait for more, but nothing comes.

“How did that happen?” Robby pushes carefully. 

“I don’t know. Just felt like it.” 

“Ellie,” Joel warns. The girl’s refusal to acknowledge something serious isn’t new to him, but it is infuriating. 

“That’s okay,” the doctor reassures him, before returning his gentle gaze to Ellie. “Did someone tell you to do this?”

She shakes her head. The doctor leans closer.

“Listen. We’re gonna help you with this burn.” Robby soothes the healthy skin of her arm with his thumb as he talks. “But I would very much also like to make sure it doesn’t happen again.”

Ellie finally speaks without looking at anyone.

“He said it was for my own good.”

Robby stills.

Joel’s breath leaves him in a rush. “What?”

Ellie’s voice is flat, distant. “At the youth club. David.” Her words are rushed now, following each other like tumbling dominoes. “He said I was special. Like him. That God put us there together for a reason.” Her fingers curl into the white sheet of the bed beneath her. “He made me do things and now my skin hurts and itches everywhere and I just wanted to feel clean.”

The room suddenly feels too small. 

Robby nods slowly, carefully. “Thank you for telling us, Ellie. You didn’t do anything wrong.”

Joel stands so fast the foot of the bed wheels sideways. “When?”

Robby raises a hand, firm but understanding. “Sir. I need you to sit down.”

Joel doesn’t know if he can. He looks down at Ellie, hands shaking.

Her face is pointed at the floor, but Joel sees the denim of her jeans turn darker where her tears drop and soak the fabric. In that moment he wants nothing more than to hug her; his instincts scream to wrap his arms around this little girl in pain and rock her until it fades. But he knows she’ll push him away, she always does, so he sits softly back down beside her instead.

“I’ll be right back,” Doctor Robby announces, and steps into the hall, leaving Joel and Ellie alone. 

Though only a curtain separates them from the incessant beeping alarms and buzzing machines of the rest of the emergency room, the sounds are all drowned by the drum of Joel’s heartbeat thumping in his ears. He rubs his sweaty palms on his thighs, resisting the urge to pinch himself. 

“Why didn’t you tell me?” he begs, trying his best to mimic Doctor Robby’s calm and tolerant tone, even though it sounds to him like he’s speaking underwater. 

The quiet drags, but as Ellie finally takes a shaky breath and Joel thinks she’s about to speak, Robby is back with Doctor Jack.

Jack doesn’t say anything at first. He just stands in front of them, arms crossed, eyes on Ellie, reading the room.

“This part’s important,” Jack says finally, voice steady. “What happened to you matters. And what you did to yourself—” he gestures gently at her arm “—is something we can help with. But I need you to understand something.”

He shifts his weight, then deliberately lifts the hem of his pant leg, revealing the prosthetic beneath.

Ellie’s eyes snap into focus.

“I lost my leg,” Jack says simply. “Different reasons. Different choices. But the same outcome.” He meets her gaze. “You don’t want to lose your arm.”

Ellie swallows hard.

“No,” she whispers.

Jack nods once. “Good. Then we keep it.”

He walks the two steps over to Ellie and puts a hand on her shoulder. Joel sees him lean down and whisper something in her ear, and he can’t be sure, he never is when they speak to his right, but he thinks what he hears is “need both paws to give ‘em hell”.

Robby takes over again, explaining next steps—social services, a forensic exam, mandatory reporting. He’s careful to include Ellie in every decision, to slow everything down.

“We’re gonna have you talk to our amazing social worker, and then you can go back home with your dad.”

Joel waits for Ellie’s usual denial, but she stays quiet. He knows it doesn’t matter, not to him, not to her (not really), but it might matter to the paperwork, so he clears it up himself.

“‘M’not her dad.”

Robby looks up. “Oh. You’re her—?”

“Foster. Temporary. Emergency.” Joel’s voice roughens. “But she’s not goin’ nowhere.”

Ellie finally turns her head, looking at Joel like she’s seeing him for the first time all day.

He reaches for her good hand. She lets him take it.

Outside, the hospital keeps moving. Shift change complete. Day in full swing.

Between them, something fragile but real begins to settle—pain acknowledged, truth spoken, damage contained.
