How about some headcanons?
Note from Doctor Emetofiend: FPIES or Food Protein-Induced Enterocolitis Syndrome, is similar to lactose intolerance with the primary symptoms being profuse vomiting when food containing a specific anti-body affects the immune system. Symptoms occurs within a few hours of ingestion.
Milkshakes -The academy is having a special milkshake day where they give out handmade shakes for one dollar.
-Maka protests, but Soul insists on getting two of them and downs them both before their lunch break is even over.
-halfway through their next class, he feels bloated and starts burping constantly.
-Maka is furious because she knows exactly what’s coming and makes it up in her mind not to deal with it because it was Soul’s own fault.
-Soul is lethargic and can hardly keep his head up by the late afternoon. He wraps both arms around his stomach tightly, any movement provoking his stomach. He can feel the milkshakes inside him sloshing around and bubbling madly like a boiling pot.
-he tries to stifle his burps for as long as he can but one escapes in the middle of class. Some of their classmates giggle and Maka shoots him a glare, rolling her eyes.
-Soul’s cheeks turn red. He stands up and excuses himself from the classroom. Before he’s even left the room his stomach lashes out at him and he clutches it, holding back a groan. He hopes nobody sees him as he runs to the bathroom.
-he hurries into a stall and leans over the toilet rocking back and forth. His stomach hurts and he’s overwhelmed with nausea. He feels himself break into a sweat and he knows it’s all over.
-he gags once, but it has no significant effect. By this point he feels so sick he wants to puke if it will make him feel better.
-another gag and this time his stomach muscles clench reflexively and he coughs painfully. Tears form at the corner of his eyes and he pants heavily.
-one more gag and he’s done for. He heaves over the toilet and pukes up thick liquid resembling a milkshake but mixed with bile pours out of him.
-he’s able to take one breath in before he’s back over the bowl heaving up more mouthfuls of his stomach contents.
-He can tell he’s been in there for a while once he starts puking up his lunch, and soon after, his breakfast. His throat is raw from straining it so much and his stomach muscles ache. He isn’t sure what time it is but he guesses that school will he let out soon if it hasn’t already.
-it turns out he is right and he finds this out when he hears Maka’s voice.
-“Soul?” She sounds angry and accusatory, which isn’t an incentive to answer her but he is so desperate by now that he does anyway.
-“Maka-” Is all he can spit out before he gags again tirelessly. She pushes the door open and just shakes her head when she sees Soul clinging to the toilet with a pathetic look on his face. “Honestly Soul, you never learn.”
Ice cream cake and pizza party? -It’s Black Star’s birthday and everyone is invited. There’s enough ice cream cake for everyone to have seconds and enough pizza for everyone to have thirds.
-Soul is pretty content sitting at the table with one piece of pizza in each hand, switching back and forth between bites.
-Maka is too busy to give it much thought. They have a test tomorrow and Professor Stein said he would be cracking down on everyone. She sits beside him, eyebrows furrowing, trying to focus with all the noise.
-After they eat, they play party games, and by that they mean, video games. Maka doesn’t mind, at least it keeps the boys occupied.
-Two or three hours go by and Maka is amazed by how long the games can hold the boy’s attention. All except Soul, who is sitting out, not watching, but staring at the wall.
-Maka closes her book and walks over to him. “Hey Soul? How come you’re not playing?” She asks, sitting next to him.
-“It’s not my turn.” He says dully. Maka thinks she should be satisfied by that answer but she isn’t. She resolves to leave him alone for a while but decides to keep an eye on him.
-Things are going well, until she looks up and suddenly finds that Soul has vanished. She looks all around but doesn’t see him with the boys or at the snack table. She figures he must have gone to the bathroom so she lets it go for the time being.
-after a good ten minutes of him being gone however, she begins to worry. She starts asking around if anyone has seen him but the boys are too focused on their game to notice and the girls have no clue.
-starting to worry now, she searches all around, only to find him ages later outside the house. “Soul!” She runs towards him, planning on chewing him out for disappearing, until she sees the state he’s in.
-he’s pale and dripping with sweat as he leans against the side of the house. “Soul? What’s wrong?” She tries not to expose the panic in her voice.
-“I don’t feel good.” He says with a wavering voice. “Why don’t we just got home?” Maka suggests, but Soul protests. “I can’t-” He mumbles, before he starts coughing suddenly. He turns over on his side, arching over the wall of the house, holding himself up with his arms.
-“Soul can you tell me what’s wrong?” Maka kneels beside him. “My-stomach-ach-it’s killing me.” He clutches his stomach with a moan.
-Soon his coughs turn into gags and Maka finally understands. “Alright, it’s okay Soul.” She rubs his back but he is too distracted to notice.
-He gags forcefully, and is sent into a coughing fit after each one. Finally he starts to feel his stomach contents coming up and one more strong heave is enough to bring it all the way up.
-He begins to vomit, some of it splashing against the side of the building. He lurches forward with each flow of liquid and Maka has to keep lifting him back up so he doesn’t fall into his own sick.
-She isn’t sure how she’s going to get Soul home in this condition, but she doesn’t intend to let him keep vomiting behind the side of the building.
-She goes inside and tears Black Star away from the video games long enough to explain the situation to him. He tells Maka she can bring him into the private bathroom in his room.
-they only just manage to sneak past everyone without them noticing and finally find themselves some privacy.
-Soul is hunched over himself in a kind of seasick stupor. Maka feels bad for him and hates being so helpless. She sits beside him and strokes his back gently. He burps several times, still leaning over the toilet, massaging his stomach.
-the burps start to get deeper and one ushers up a gag. Soul jerks forward over the toilet and vomits up more fluids into the water. He comes up for breath, looking at Maka with droopy eyes. “Maybe ice cream cake was pushing it.”
