Roll a Wisdom save. A character with established anxiety/trauma or triggers related to the situation must roll with a DC of 13 while a character who might be affected less must roll with a DC of 10. If a character fails, roll a d10 to determine how long the attack will last (if they rolled a natural 1, then they automatically suffer the consequences of the attack for 10 minutes) and roll a d10 on the following chart for every successive minute of the attack.

1- Heart Palpitations. The affected character becomes acutely aware of their racing heartbeat and it becomes all that they can focus on. Roll all Perception, Investigation, and Insight checks with disadvantage.

2- Shaking Hands. The affected character’s hands begin to tremble uncontrollably. Roll all Dexterity and Dexterity-based checks with disadvantage.

3- Nausea. The affected character feels as though an entire nest of spiders is crawling in the pits of their stomach. Roll a Constitution save (DC 10). If the character fails, then they are incapacitated for the next minute as they try to control their upset stomach.

4- Shortness of Breath/Choking. The affected character feels as though they are at the bottom of the ocean and the pressure and lack of air is compressing their chest. Roll all Constitution- or other breathing-based saves with disadvantage.

5- Chest Pain. The affected character feels as though a hostile creature barreled directly into their chest. Add 1d4 psychic damage for any physical damage they incur during the attack.

6- Sudden Onset of Fear. The affected character feels overwhelmingly frightened of something intangible and indescribable. Speed is halved and all saves against being frightened must be rolled with disadvantage.

7- Depersonalization/Derealization. The affected character feels detached from the world and struggles to return to their proper mind. Passive Perception drops to baseline 10.

8- Vertigo. The affected character feels dizzy and seems to be on the verge of falling over. Speed is halved and all Intelligence-based checks are rolled with disadvantage.

9- Sweating. The affected character begins to sweat profusely, no matter what the climate. Add 1d4 psychic damage for any cold or fire damage they incur during the attack.

10- Sleeplessness. The affected character is dually extremely wired and extremely tired, vacillating between the two. For every minute that the character undergoes, they gain a level of exhaustion.

The character can choose to spend five minutes calming themself down. Roll at the beginning of the five minutes to attempt to save again. 

Upon a save, all symptoms of the attack fade.
