DISCLAIMER: This post and all the points I make are highly based on my OWN experiences. I often find parallels between my experiences as a disabled individual and characters I love to help me better cope with and process my feelings. Hate will not be tolerated!!!

Before I get started, I’d like to say that this is not even me scratching the SURFACE of the things I could analyze about Leon and apply to various autistic experiences, this is mostly just the things that resonate with me the most.

I wanted to use this as an archival sort of post to document any instances in which we were shown or told that Leon was injured in some way. But for purposes of canon accuracy, I'm not going to be including the original games (meaning no cheek scar or stab wound from OG4, etc.) nor will I include any speculation of injuries. There are a lot of times in the franchise where Leon was undoubtedly injured, especially in instances where he was tossed by BOWs, but unless there was a clear visual cue like in Vendetta with him holding his arm afterward, I'm not going to really speculate on it since there's really nothing concrete to go on.

RE2

GSW

The biggest, most obvious injury he sustained here is a gunshot wound (GSW) to his left shoulder. It's hard to tell exactly where the bullet hit him due to the motion blur, but based on the blood-spray from the back part of his shoulder and the blood on the wrap in the front, there would be a clear entrance and exit wound because it went all the way through.

Based on where the dark, more concentrated blood staining is on the wrap, it looks like the bullet almost definitely hit some sort of bone. An exact answer would be heavily dependent upon the entrance and exit angle, the position his arm was in, etc. which we can't see in its entirety, so this is my best estimate. So, it looks like it could've potentially his his scapula or his clavicle.

(Also, note that the placement of where the blood is seems to change a little based on if Leon's wearing the OG2 version of the police costume or the RE2R version. )

But, needless to say, that's going to leave a pretty decent scar and depending on what else was hit/damaged, he likely undoubtedly had to have surgery as well, so there's the potential for surgical scars as well. For example, if the bullet had fractured his clavicle and there were lose bone fragments, they would've had to have done surgery, so he would've had an incision near his collarbone.

RE4R

Concussion

First, he gets slammed so hard he loses consciousness, so that's at least a mild concussion. It's unclear how long he was out for so it's hard to classify it, but it's seemingly at least a mild of moderate TBI.

Fuck up Right Knee

Next, it's not clear what exactly he would've done to his knee here, but slamming it between a metal chain and concrete floor is gonna at least bruise pretty badly. We can see that he limps afterward with the freecam, so rip Leon's right knee.

Cut on Right Hand

Then, Ashley slices his right hand while she's possessed. There's a pretty decent sized blood spray and it's enough that it momentarily fazes him and he looks down at his hand, groans, and holds it so there's definitely going to be a scar there where the blade got his skin.

Laceration to Left Thigh

After that, Krauser slices his thigh pretty good. His blade initially hits the inside of Leon's left thigh and then cuts across the top of it. Again, pretty big blood gush there, and Leon struggles to walk when he goes over to Luis afterward, so Krauser got him pretty good. It doesn't look like it was deep enough to hit his femoral artery, but it's definitely going to scar, and it's gonna hurt like hell.

Damnation

Now, Leon really took a beating in Damnation, and while it's mostly just implied in the movie, we get a lot more information on his injuries in the novelization. It even specifics that he was in so much pain, he laid on the ground throwing up at one point. He also nearly died, so this is definitely one one of the worst missions Leon's been on, at least that we've seen.

Concussion

Like usual in this franchise, Leon gets slammed around a lot, but just like in RE4R, he hits his head after being caught up in an explosion and then falls unconscious a few moments later, so he likely has some degree of a concussion, especially considering he has a headache when he regains consciousness.

Minor Cut to Throat

After Buddy and JD successfully capture Leon and tie him to the chair, Buddy holds the tip of the knife against Leon's throat, right under his chin, after Leon makes a sarcastic comment. Note that this is not the part where Buddy holds the knife to Leon's throat while covering his mouth.

Fractured or Broken Ribs and Clavicle & Dislocated Left Shoulder

During Leon's final fight with the Lickers and the Tyrants, he got tossed and slammed around a lot, which resulted in his left shoulder being dislocated, one of his collarbones being fractured (likely the left one), and several of his ribs likely being fractured.

Unspecified

Although it's unclear exactly what injuries Leon sustained, it's mentioned that he has a lot of pain in his arms, legs, head, and waist apart from the other areas previous mentioned, and considering the fact that he nearly died, it's likely a bit more than some bruising.

Vendetta

Potentially Dislocated Right Shoulder/Injured Right Arm

Based on the ending of the movie and when Leon approaches Chris, he's holding his right arm, signaling that it was injured. A lot of people have speculated it was probably his shoulder that was hurt, and that seems to fall in line with what's shown in the film as well. Arias picked him up and squeezed him twice, and both times, it looks like Leon's shoulders are forced back, which would put a lot of strain on them. This could mean his shoulder was dislocated, or it could've been a muscle tear or strain, rotator cuff issue, etc. So while it's unclear exactly what happened to it, he definitely got hurt enough so that he's holding it.

would definitely thrash and cry in his sleep sometimes, the amount of horror and gore he’s experienced first hand wouldn’t be taken away just cause he has a pretty thing like you to love.

i feel like he’s always so detached and constantly disassociating that whenever you try to initiate something, anything at all, he just sort of does it out of inertia, just so you can get the relief you want.

OK WAIT. BECAUSE I'VE THOUGHT ABOUT THIS A LOT.

I don't know enough about astrology to theorize an entire birth chart for Leon, however I do have a headcanon that Leon's sun sign is Pisces based on two things:

The fact that Leon's already 21 years old on September 29, 1998, meaning that he had to have been born before then, given that he was born in 1977.

That Pisces is the sign known for being immensely empathetic due to them being the last sign of the zodiac (so they can relate to/have adopted traits from all other zodiac signs). If you look up Piscean traits, every single website you find will mention Pisces' compassion/empathy — it's the primary trait associated with this sign. They're also known to be adaptable, personable, self-sacrificing, idealistic, and to engage in self-destructive escapist behaviors (i.e. drinking heavily) due to their inability to accept the injustices of the world around them.

Some quotes about Pisces that perfectly describe aspects of Leon's personality:

"Easily touched by human suffering, at least in theory, Pisces wouldn’t hurt a fly. They believe in people, are deeply hurt by compassionless human behavior, and have a hard time saying no. Harsh realities are avoided either through escapist behavior or self-delusion." (x)

And if they struggle to set boundaries, they might feel like they're constantly being taken advantage of for caring about and for others. They might be inclined to paint themselves a martyr.
