in all seriousness, i do 100% believe that Splinter is the only one who can immediately and accurately tell them apart without any identifying colours or weapons. after all, he did raise them!!

and i headcanon that after awhile, eventually April can too. it's way easier to tell them apart if they've got relaxed/natural facial expressions, especially with Raph and Mikey. (if you closely watch when Raph is maskless, he has SUCH Raph-y expressions lmao. usually pissed off ones.)

if they want to really test April (by keeping their faces completely blank), then she goes by skin tone, because they do have distinctive shades of green. Donnie and Raph are easiest, being the lightest and darkest greens respectively. Leo and Mikey seem trickier (at least to my eyes) because at times, Mikey's green looks similar to Leo, but in some darker scenes, Mikey looks closer to Raph.
