It's interesting how Sukuna's true form has ear guages. In some buddhist beliefs, the earlobes were elongated in methods similar to modern-day ear guaging to better allow the person to "better hear the voices of the suffering". Why would Sukuna of all people have these?

Geto had them, and it makes sense for him. He himself was suffering, and by the time he left, his ears were pretty stretched out. It makes sense that the man suffering and this suffering being neglected would not want anyone else to be neglected like he had been.

But Sukuna is the King of Curses. A brutal, violent, destructive, immoral, sadistic cursed object. Why would he of all people wear such a symbolic item of empathy?

While a symbol of empathy, it's not necessarily restricted to such devoted reasons behind it. It's to better hear the suffering of others. While this may seem noble, perhaps Sukuna just likes listening to the suffering. He's cold-hearted and apathetic, so he probably gets a kick out of using this spiritual symbol and twisting its meaning to better suit his immoral ideals.
