i think it's funny when a character is almost exclusively referred to by their last name by other people and this carries over into their internal monologue when someone writes from their pov. not even on a first name basis with herself

Heated Rivalry fandom does this, I wish they wouldn't

I'm not sure I really understand the problem with this, if we're talking third person. Like most people, I'm called by several names by different people, but I don't think of myself as any of them particularly more than the others when I'm in my own head. Unless I have a good reason to, I prefer to stick to calling a character by one name in a story for the sake of consistency, it would feel self-conscious and noticeable to chop and change. But it does depend on the character and the story.
