Rejected Case Notes

Client would like to know why SW has not contacted agency client is working with for housing. SW reminds client that client must sign ROI for SW to contact the agency. Two months later, client has not signed ROI.

Client died due coronary event. Client could not receive oxygen prescription. Due to homelessness. Client died due to homelessness. 

Client approaches SW asking “how bad” wounds on arm look.  Client squeezes one pustule which leaks.  Client does this over SW’s salad.  SW does not continue eating.

Client does not show up to scheduled appointment.  Client shows up 4 weeks later wanting a same-moment appointment and cannot be accommodated. 

Reception contacts client to confirm appt.  Client complains about time.  Client states “I might show up, I might not.”

Colleague in a public setting has brief HIPAA violation.  Laughs about it. Becomes extremely defensive when it’s pointed out.

Client requesting assistance with housing subsidy application.  Client believes that they will be able to access said subsidy with alacrity.  SW informs client of waitlist.  Client does not believe SW, states they will get “a second opinion” from a for-profit organization.

SW is scheduled to meet with Client A at the same time they are schedule to transport Client B.  SW has attempted, but cannot exist in superposition. Clients displeased. 

Client speaks slowly and haltingly in phone message, words interspersed with frequent linguistic fillers (e.g. “um”, “uh”).  Client then relays phone number with extremely rapid speech. SW forced to listen to message multiple times to retrieve phone number. 

Client leaves long message about specific assistance needed, declines to leave phone number

Client of color asks SW if not wearing socks in the winter is a white person thing. SW states, as a white person, that it may just be normal teen behavior. Client denies this possibility, insists it is a white person thing.
