Sometimes patients fail to disclose something obvious but important.

When suspected, maybe take some time to tease out the relevant information.

GCS score can be indicative of how critically ill patient is.

A declining GCS is worrying sign in any setting and should warrant prompt assessment of the airway.

If GCS < 9, there is clinical concern for airway protection and intubation can be considered.

You try to show off, but there is always one of your colleague.. 😅

Simple ABG interpretation using mnemonic 'ROME'

Reference normal range: pH 7.35-7.45 PaCO2 4.7-6.0 HCO3- 22-26 Base excess -2 to +2

Step 1: pH low or high will show acidosis/alkalosis Step 2: Compare pH and CO2, if they go opposite direction, then respiratory acidosis/alkalosis Step 3: Compare pH and HCO3-, if they go same direction, then metabolic acidosis/alkalosis Step 4: If both Step2 and Step3 are positive, then mixed acidosis/alkalosis

Obviously there are more in ABG such as Type1/2 resp failure, anion gap, base excess, but mnemonic 'ROME' will give you a quick guide in a nutshell 😉

Suspect Drug Seeking Behaviour When: 1. asking for specific med 2. 'doctor shopping' 3. prescription/med lost 4. pt might have an excellent knowledge on controlled med This is few examples of many.

However it's important to be objective, and not judgemental. Pain is also subjective.

* understand the information

* weight the risks and benefits to make the decision

* be able to communicate the decision

NHS was born on 5th July 1948.It has been saving countless lives in UK over the last 70 years. 

It's not perfect and has got flaws.However the failure isn't caused by itself, but by bad policies and the government. Cutting nursing student budgets, frozen NHS pay and driving healthcare workers away from the system, #Brexit causing skilled overseas doctors/nurses coming to work while lack of home-grown workforce, just to name a few..

I'd like to see NHS prosper and continue to serve for people.I am grateful we have NHS and proud to be a part of it. Happy birthday
