PCOS has a new name - Polyendocrine metabolic ovarian syndrome, or PMOS for short.
The summary from The Lancet (12 May 2026): "Polyendocrine metabolic ovarian syndrome (PMOS), previously named polycystic ovary syndrome (PCOS), affects one in eight women. However, the term PCOS is inaccurate, implying pathological ovarian cysts, obscuring diverse endocrine and metabolic features, and contributing to delayed diagnosis, fragmented care, and stigma, while curtailing research and policy framing. Building on an international mandate for change, we outline an unprecedented, rigorous, multistep global consensus process for the name change. Funding and governance were established with engagement of 56 leading academic, clinical, and patient organisations. Using iterative global surveys (with responses from 14 360 people with PCOS and multidisciplinary health professionals from all world regions), modified Delphi methods, nominal group technique workshops, and marketing and implementation analyses, we identified principles prioritising scientific accuracy, clarity, stigma avoidance, cultural appropriateness, and implementation feasibility. An accurate new name was prioritised over retaining the PCOS acronym or a generic name. Implementation approaches prioritised evolution rather than transformation. Preferred terms were polyendocrine, metabolic, and ovarian, reflecting the condition's multisystem pathophysiology, and polyendocrine metabolic ovarian syndrome was the consensus new name. Accuracy was improved by omitting cysts and by capturing endocrine, metabolic, and ovarian dysfunction. A co-designed global implementation strategy, including a transition period, education, and alignment with health systems and disease classification, is under way."

The term polycystic ovary syndrome (PCOS) has long been recognised as inaccurate and potentially harmful. The following evidence-based considerations informed the need for a new name:
• The term polycystic ovary implies the presence of pathological ovarian cysts, which are not a feature of the condition. This misnomer contributes to misunderstandings among patients, clinicians, policy makers, and the public.
• PCOS encompasses diverse endocrine, metabolic, reproductive, psychological, and dermatological features. The current name reflects only one organ and fails to capture the disorder's multisystem nature.
• Confusion arising from the current name can delay diagnosis and hinder effective communication between patients and health professionals, contributing to patient dissatisfaction with care.
• The reproductive focus of the name can reinforce stigma, particularly in sociocultural contexts where fertility carries high value. Many individuals report distress associated with the name itself.
• The misnomer complicates epidemiological classification, research comparability, and health system coding. A more accurate name is expected to improve scientific coherence, research funding, and policy alignment.
• International guidelines, expert groups, and patient organisations have repeatedly called for renaming, with serial surveys and workshops culminating in a mandate to change the name through a rigorous, global consensus process.
• A new name must support long-term clinical care, research, and global adoption, and enable a smooth transition from existing terminology.

Learning this was an intentional genocide changed me.
I know most of those following me know this, but just to make it super clear. An Gorta Mór (The Great Hunger/the Great Famine) was a deliberate genocide of the Irish people. There was enough food grown in Ireland to make sure everyone was alive and healthy and survived. Instead it was exported, sent to England and elsewhere for profit while men, women, and children starved in the streets. While the English landlords fucked off and evicted starving families who couldn’t afford rent. While babies were too weak to cry and died at the side of the road.
They tried to kill us, but they did not succeed. And we owe so much thanks to the other oppressed peoples, in particular the Choctaw Nation and the Masai, who sent money and grain to us.
Let me repeat that. The Choctaw Nation who had just gone through the Trail of Tears sent us money to try save Irish lives. It’s led to an understanding between Irish people and Native American tribes, most recently when we donated to the Navajo and Hopi fundraisers for COVID-19 relief, because while it may be a different tribe, Irish people will never forget those who helped us and we’ll help back.
The entire population of the island is less than seven million people. We’re still a million less on this island than pre famine. And it’s not that long ago. My grandmother’s grandparents lived through it. We’ve told the stories, it literally changed the DNA of the country. We have a national fear of renting, because so many people were evicted. People joke about Irish people always offering loads of food, but it’s because there’s that cultural memory of not being able to.
They tried to kill us, but they did not succeed. We will not let them take our lives, we will not let them take our language. We lost so much, but we will not lose it all.
This is why I get so angry when people say “it was the potato famine, it was because of monoculture/microbes.”
Nope. The potatoes were the only thing Irish people were allowed to fucking eat, because as pointed out, the rest of the crops they were growing were for their landlords to ship to England. So when the one “worthless” crop they were allowed to eat rotted in the field, the English crown, empire, landlords, all shrugged and carried on. People starved to death lying next to productive fields.

This is legitimately useful reframing. A while ago I started replacing the word "cop" in my vocabulary with "a man with a gun." It really puts things into perspective.
This homeless person is making me uncomfortable. Should I call [a man with a gun]?
My neighbor is having a loud party. Should I get [a man with a gun] involved?
There are some teenagers skateboarding. Do you think [a man with a gun] would get rid of them for me?
It makes it very clear what you're saying. I can call a man with a gun to threaten or hurt someone mildly inconveniencing me. You're not calling the cops, you're calling A MAN WITH A GUN into a situation that does not warrant a firearm handled by a volatile lunatic who will not be held accountable for his actions.

Except, critically, at bedtime.
Ravings and urges get miscoded over time. Let’s say you’re thirsty, and you live in a strawberry field. Strawberries contain some water and a bunch of sugar so, over time, you may start to crave strawberries when you are thirsty because you get a reward and some relief in shorter time from the need starting than the trek to the stream. This can happen for every need: sleep, food, whatever.
Trevor Noah has a great tip, that when he craves ice cream at night he breaks it down into parts: I want something cold, I want something sweet. He drinks a glass of cold water then waits to see if he still has the ice cream craving. Usually he doesn’t.
So listening to your body isn’t “follow every urge” but “decompose the urge to discover the underlying need.”
If you always feel like getting cozy in bed you may be: cold, dehydrated, and/or malnourished (maybe a need for high calories that are bioaccessible…not processed).
If you do not feel tired at bedtime you may: need to eat dinner earlier because your body is still digesting, need to exercise or go outside more during the day, get the fuck off your screen for an hour so your brain can enter sleep mode.

P.S. notice i said nothing about neurodivergence. Not that it’s not a likelihood but the over-pathologization of behaviors prevents us from taking simple actions to improve our wellbeing. Also, these tips are pretty accessible and applicable to most brain variations.
