Like hear me out, he was born with "male anatomy" but was assigned as female at birth because of moon bulshit, so he was raised as a woman.
And when he was tasked to take care of Anor Londo when Gwyn and the other gods fled the city, he also made the illusion of Gwynevere to... idk probably just keep him company or help him control the 3 people that still lived there, honestly Dark Souls lore is already convoluted as is so give me a break.
But here is the thing, this copy of Gwynevere was created by Gwendolyn and did everything he wanted since she was just an illusion and her true self was god knows where (probably fucking with another dragon if we are being honest, good for her tbh), so he was fully aware and deliberate when he made Gwynevere call him her "little brother" and use he/him pronouns to describe him, there is no way he would let that slide if he didnt want to.
So, to sum it up: Gwendolyn was born with "male anatomy", assigned and raised as a woman because moon = girl I guess, and when given the chance to be called anything he wanted by his creations, he chose to be reffered as a he and to be a little brother to his sister.
Does this make sense? Is this crazy? Am I crazy? Maybe a little bit tbh.

I am not sure if this thought came across someone else here before, but here’s my theory to why Will held on to Hannibal and threw them off the cliff:
Okay so, Will standing there having his moment with Hannibal, and we, the audience don't hear it, but what they hear are sirens. Let’s not forget this was always Will’s design. He planned this from the beginning, he was always going to "breakout" Hannibal from prison. I am 100% sure he accounted for Francis to interfere, and that they would end up in that hideout. What I think came as a surprise to him was the death of all these police officers but then again he doesn’t really care. So anyway, Jack always knew about this plan, whether Francis interfered or not, the plan was exactly the same. I am certain there was a tracker somewhere on Will to keep Jack on the follow up. Don’t forget that Will’s plan was that him and Hannibal kill Dolarhyde first, and then Will kills Hannibal. And again I am sure that's why Jack did not interfere right from the get go, and let this happen. He knew this was the only way it could happen. Now that the dragon was gone, he can show up to “rescue” Will after of course he’s murdered Hannibal. But all he sees is Will holding Hannibal and throwing them off the cliff. I can picture how Jack screams “Nooooo” and runs up to the edge but sees nothing from the darkness that engulfs the two bodies. This was the only way they can be confirmed to have “died”. To make sure no one will ever come after them again. Once again, this was always Will’s design, meaning he prepared every move before hand and accounted for the fall. What do we know about Will? He’s one hell of a sailor and has his own boat. Meaning down the cliff a boat is ready for them. And yes surviving a fall that deep seems impossible but he believed they could make it if they held that tightly. For once, he wanted to be the one to surprise Hannibal.
