This is a clinical genetics intake form, not a general ethnicity survey. I'd bet it's at an OBGYN office.

The form isn't doing an ethnic survey or trying to get to know the patient, it's trying to figure out if the patient needs Ashkenazi-specific genetic carrier screening - it isn't forgetting or invalidating anybody.
