A friend and I were in a debate about whether or not Carmy knows how he feels about Sydney. I couldn't articulate my reasoning, but this is the main one that gives me the feeling that yes, Carmy knows, and he has known and was certain in Season 2x9 with his panic attack ending by the thought of Sydney walking into his life twice.

And the season finale tells what happened in the panic attack, it was Richie and Mikey pushing Carmy to see Claire as the good thing and then once his thoughts settled he thought of the actual good thing. As he states in the season finale any kind of good started when you walked in (hi hello, Carmy's panic attack ceases) and any chance of it surviving is with you (thought of Sydney walking back in his life keeping the good thing going and surviving in his family).
