SO! It's been a while since I open my physiology book, especially about diving and submarine physiology! I'm so pumped to answer these.
I read my anesthesiology book and journals, watched submarine video because at first I have no idea about human in submarine physiology, asking 2 residents (resident is a doctor who took another year in hospital to become a specialist, in this case I'm asking Anesthesiology and Surgical resident).

Let's jump, this is from episode 489. The clue "both of them are bleeding uncontrollably"
I presumed Luffy and Jinble are on hemorrhagic shock grade III / IV , and to differentiate between these two, we need the exact measurements of blood loss, blood pressure, heart rate and respiratory rate, so lets just assume both of them got grade III / IV hemorrhagic shock because the only clue we have here just their state of unconsciousness resulted from multiple injuries the got from fights in Marineford.
The scene that causes me a question, awww, the answer it is POSSIBLE! to do the surgery without anesthesia. Because they are on an ongoing bleeding, Law have to do a surgical resuscitation or often called
Damage Control Resuscitation
The main purpose of Damage Control Resuscitation is literally to search the cause of the bleeding. To stop and control it. With formal definition is to prevent lethal triad: coagulopathy, metabolic acidosis and hypothermia.

To prevent Luffy and Jinbe from death.
Law here talked about pain referred to what he said "they are both seriously hurt that they can't feel any pain" handled by one of the " analgesia " principle in anesthesia triad —hypnosis, analgesia and muscle relaxation. Also, in this case the hypnosis principle is already " done " due to their injury a.k.a hemorrhagic shock. But back again, anesthesia not only matter of pain, but also those three above.
I'm asking one of the anesthesiology resident about this too, and he said it's not ethical not to give a trauma patient anesthesia, not the matter of possibility 😂
