I think that Spike was changed long before he got his soul back. While he was always capable of love (in fact his love for Dru got a demon they summoned back in season 2 to tell them that they "stink of humanity") he wasn't capable of being selfless until Buffy and Dawn. At a certain point he started really caring about not only Buffy, who he was in love with, but also Dawn who he had no reason to care about.
This was a genuine moment of empathy and care, and you can tell because he backed out because HE was also vulnerable. Vampires aren't supposed to be able to do that.
