Headcanon: Voldemort refused to smoke cigarettes because they're lethal, and he looked down on everyone who willingly shortened their lives by smoking. After creating his 6th horcrux, however, he started smoking occasionally just to brag about it not affecting him.
He was very careful with alcohol, too, because he avoided losing control or becoming too vulnerable due to weakened cognitive functions.
Bellatrix used to be a heavy smoker until Voldemort scolded her for having such an unnecessary self-harming habit. After that, she only smoked in secret, usually when Voldemort gave her the cold shoulder.
She loved drinking excessively, especially Firewhiskey.
