Ok trans orbital lobotomies were invented in the 1930s however the first psychosurgery procedure was performed in 1888 so his uncle would be ahead of the medical community in adopting this practice here. (They didn’t use icepicks at the time so my art is kinda inaccurate but shhhhh ) 

Interestingly at the time this procedure was not received well by the medical community, it would be interesting if Giyuus uncle was punishing him in a way or if their family was just that desperate for psychiatric help they would resort to experimental treatments. (Perhaps as a way to save face?) 

Also sorry to my radiology professor this drawing isn’t medically accurate, i gave up below the neck 😔. The colours also don’t mean anything i choose them purely for aesthetic also I choose a mid sagittal ct cut because a left cut looked a bit ugly.
