My vision for this is Dirk laying face down with his shirt off and Dave sitting on his lower back, starting with the scalpel right at the base of his neck and stabbing into there. Cutting along the curve of his spine, going back up and slicing along his shoulder too so he can start peeling back the skin.
This painting always struck us in a very horny kind of way, I'm imagining something like this for how he gets cut open but cut close to the skin.
