It could be either for social transition and/or medical transition. Usually someone who socially transitioned already, they may also use "pre-t", "pre-op", "pre-HRT"... Someone who didn't transition in any way may also use "pre-everything".
Based on the transitioning flag. It shouldn't be confused with nontransitioning (non-HRT, non-op...), which is the state of someone who isn't willing to transition.
