I think that happens if you get his health too low in the second-to-last phase. I’ve always stopped dps at 50% when he transitions, but I haven’t killed him very recently so maybe it’s a diff bug.
I'm pretty sure the issue was that we were both MCed during the phase transition and just had our respective minions beating on him. He was at 52% health.
