Sam is actually Lucifer (or Samael before the fall), but he doesn't remember it. On May 2, 1983 (Sam's birthday), Lucifer was able to escape the cage. His grace remained in the cage, but his essence ended up in Sam's nascent body. Upon entering his true vessel (Lucy didn't intentionally possess Sam), Lucifer's memories vanished, and his essence simply merged with the nascent soul, which hadn't yet formed. So he wasn't, so to speak, an adult in a baby's body or anything like that. No one knew about this. After the cage was opened, instead of releasing the devil, his grace was released and returned to its host. Sam doesn't recognize his grace, because he doesn't remember himself as Samael.
