also, even with those lacking empathy, there's still a high chance of them having cognitive empathy instead. someone could be extremely emotionally sensitive, describe themselves as an "empath" and still potentially meet the DSM criteria for NPD. but with the emotional attunement turning inconsistent when shame, criticism, rejection, or threat to their status is present. to be honest, as someone who's a communal narcissist, i get gratification through being moral, caring, selfless, and emotional intelligence because claiming to be highly empathetic makes me feel as though i have power over other people's emotions, and get supply through others seeing me as a good person. those who call themselves an empath COULD be using it as a grandiose self label, grandiosity can be disguised as humility, feeling entitled to emotional access in other people, wanting narcissistic supply through being a "caregiver or saviour", wanting immunity from criticism by being "an empath." what i mean is that, while healthy empathy can be present in those with NPD, while genuinely having good intentions, it can have negative impacts because of emotional instability and their narcissism affecting them. feel free to add stuff or correct me
