But as a non-salaried doctor whose inbasket AND schedule are always overflowing, I have NEVER ONCE considered finances and the need to fill slots or bill more when deciding whether someone needs an appointment or not.

What I have considered and wished I did not have to is CYA. Can a stickler claim that I did not exercise due medical diligence because I did not actually see the problem with my own eyes or hear the patient’s symptoms with my own ears.

With an in person visit I always have more security in saying yes, I exercised due diligence because I had time and opportunity to gather all possible information and there were no intermediaries.

Why is it important? Because of the culture of litigation*. That’s why we do half of what we do in medicine. That’s why documentation burdens are so high. That’s why malpractice premiums exist. That’s why so many unnecessary tests and procedures are done. That is why you sometimes can’t make a meeting an email, even if it seems like the common sense thing to do.

(Also, sometimes laypeople don’t know what they don’t know, so while a patient may think it’s appropriate to make a meeting an email, there are legitimate medical reasons why they can’t. But that’s a different story).

Somehow, with the hobbits there, it had felt like there was something left to fight for. That after what he had done to Frodo, he could atone for his sins. Now that was gone. His breath was shallow in his chest – it must have been that way all that time, but he never noticed. His limbs were heavier still – each like ten boulders, in need of strong horses to pull them. His vision rippled, its edges closing in. Everything was either very dark or very bright. He felt pain, but it was not part of him.

The bowman stood before him, a white, hungry smile on his grime-caked face. He raised his bow and pulled a final time.
