The development of this greatly prospered some time into the Alicorn sisters’ reign, when they enacted laws that required Unicorns within the army to wear armor specifically crafted to protect their horn without limiting their magical capabilities ( often enhancing them, even ). As a result, the numbers of soldiers sustaining horn-related injuries lowered drastically.

However, as ways to safeguard horns became more sophisticated and thus common, medical and magical knowledge to treat them lagged behind. Although considered almost unheard of now, broken and otherwise damaged horns exist to this day, whereas finding a pony knowledgeable on how to handle them ( or powerful enough to restore one* ) proves difficult. It is a subject still taught, but few truly specialize or otherwise research into. 

The most reputable medics for expertly handling such injuries are in direct service of the princesses, seeing to the needs of Equestria’s Alicorn rulers and their royal guard. There are a handful soldiers who to this day have frequent appointments to check up on a horn that was damaged long ago in service of the land they swore to protect. 

* Restoring a horn is a very delicate task to begin with because of what intricate part of a pony’s very being they are, and using magic to attempt a process of mending it is nearly impossible. If truly fortunate enough, their own innate magic and what traces of it remain in a piece may reconnect and thus mend itself, but one must act fast. Few are this lucky, and Unicorns with more magical potential have a better chance, as it takes longer for the spark of magic within a broken piece to disperse.
