If we want extra angst I’d say Elsa would be… disturbed, to say the least. Her initial reaction is that of disgust, but not that she lets Anna see that. She approaches her with concern and insists she will try to find help, to get this tumor out of her. She blames it entirely on her supposed illness and not at all on Anna herself. But she’s very, very put off by it at first. She might try to teach Anna the conceal-don’t-feel technique if you really want to suffer but whether Anna buys it or not would depend on her own disgust. On the other hand at this point Elsa should know concealing-not-feeling causes more harm than good so i could only see this happening in an AU or some situation in which they find each other before the coronation. But as time passes she realizes that… this is the same Anna she’s always loved. Her little sister. She’s still brave and kind and awkward and funny a bit innocent and times and god she still loves her. No illness or incestuous perversion could take Anna away from her. I guess she kinda becomes desensitized to the horror of incest with time and begins to tolerate and even accept Anna’s attraction towards her as just another part of their affection. Because Anna is beautiful and nothing Anna does could be disgusting. Especially to Elsa. She might even start to enjoy the love and the attention, when her heart begins to skip a beat every time she sees her or when she feels warm inside at the sound of her laugh. All that blushing and awkwardness and rambling that shoved this problem into her face and that she initially found uncomfortable and off-putting are now endearing to her. How had she ever been disgusted? How could that be possible?

Ok the more fluffy side, i think Elsa would be very smooth most of the times but really the dynamic changes depending on how comfortable and confident the girls are. At first when Anna is nervous and longlingly staring at her from the distance (or from up close), sighing and smiling like the useless baby lesbian she is, Elsa might find it cute and adorable and might feel the need to ease her nervousness and provide some comfort and stability as the big sister, hence being smooth hehe. Or maybe she’s just flirting and trying to make Anna more flusteted. But as the relationship evolves and Anna becomes more secure and confident she begins to take the lead (“I prefer you in leather” ANYONE???), and Elsa mercilessly follows and blushes at her flirting.
