Caroline here; I believe the answer to this is yes, there are trailers set up at N. Gower Street for makeup and costuming. However, not being personally familiar with the setup, I would defer to @cupidford and Ruther2 and @thesetison and @penns-woods and others to describe the scene more in detail. It looks like a drug trials set up. I’ve been involved in some of them, the iv stands, the age of the patients. The garden setting as well, they try and give them nice places to sit. All ties in with Sherlock being affected by something, maybe this is the antidote scene?
Intrigued by who the doctor wearing scrubs + white coat & trousers is. Looks too blonde/thick-set to be John?
And Sherlock definitely still has the greasy hair/stubble combo.
