ICU again
I’m so tired of this bullshit. And the worst thing is, technically, my class is capped at the 6 months maximum before even starting 3rd year, because we did a month of ICU days, a month of nights, and enough call for another month (I learned that any and all call for the unit counts toward that cap) as an intern and then 3 months of q4 call in our second year. 6 months of ICU before even becoming a 3rd year. What the fuck.
But, our wonderful PD never confirmed what is counted toward that cap with ACGME and thinks we’ve only done 5 months (previously thinking we’ve only done 4 months because he didn’t count the month of ICU nights like a dumbass)
So for our third year, he had scheduled us for another joyous 2 months until we forcefully told him that the month of ICU nights actually does count. So now he thinks we are at 5 months and not the actual 6 because he never fucking bothered to confirm with ACGME what counts towards the cap (I know because I emailed them and now I get the feeling he’s ignoring the month of random call we did as an intern). His solution? To schedule us for the two months but only make us do 2 weeks out of each month because we can’t afford to be short one senior resident.
So now we’re super shorthanded to the fact where we have multiple days where there’s only 1 resident managing an almost 30 bed ICU + step down.
To make matters worse, because we’re so shorthanded, we can’t afford to not advance an intern to being a senior on ICU no matter how bad they are... Like not checking an ABG on someone who’s not intubated and on 17 levo and when my friend gets on in the AM and gets an ABG.. pH 7.1.
Long story short: I’m stuck with one of the worst teams this month and super shorthanded and I’m so anxious I want to vomit.
