I headcannon that all Minecraft worlds, realms, and servers are physically connected but are separated by farlands and all the farland types. If you are strong enough and have the will to get though the farlands then you might be able to reach a different world, realm, or server. 🙃
That’s a great headcanon! I personally imagine them as having a network that you can hop between, but the idea that you could travel through the farlands to a new world is very interesting!
