Midfacial Internal Distraction Osteogenesis for Crouzonβs Syndrome Crouzon syndrome involves the premature fusion of skull bones. The midfacial sutures are most affected by this condition. Eyes are wide set and bulging due to shallow sockets. Le Fort III osteotomy with midfacial advancement through internal distraction osteogenesis using Kawamoto distractors is the treatment of choice. This 14-year-old boy had the above presentation. Treatment planning was explained to his parents and they consented to surgery. Fixed orthodontic treatment was commenced for alignment of teeth before surgery. A bicoronal flap was raised followed by exposure of the glabella and supraorbital notch. Bone cuts were then made in the zygomatic arch, frontozygomatic suture, floor of orbit and nasion. The entire midface was then disjointed and bilateral Kawamoto distractors were fixed following detachment of the temporalis muscle. A total distraction of 18 mm was performed followed by a consolidation period of six months for hardening of the bone at the distracted sites. There was an immediate improvement in the patientβs symptoms along with normalization of his facial features. Unilateral Mandibular Distraction Osteogenesis This patient had a facial asymmetry due to a mandibular hypoplasia on the left side. There was a midline shift to the left side. She had been advised to undergo left-sided distraction osteogenesis of her mandible. Bone cut was made in the left ramus and the ramus distractor device was fixed with screws. This was followed by a Le Fort I osteotomy of the maxilla. A total distraction of 6 mm was performed after a latency period of one week. This resulted in establishment of facial symmetry for the patient. The distractor was removed after a period of four months to allow for bone consolidation at the site of distraction.
