Here’s an interim progress photo of the patient with Granulomatosis and polyangitis with the forehead flap in place. There is much more to do, but we are dividing the forehead flap from its pedicle this week. Then, the shaping will continue. This case is very challenging because the underlying disease process that did the damage is still a factor. Reconstruction of Moustache and Ala of the Nose with a Single Forehead Flap This young man had met with a road traffic accident approximately three years ago, sustaining extensive facial soft tissue injuries involving the right alar base and mustache. Surgery had been performed at a multispecialty hospital, but had left him with a severe alar deformity as well as loss of hair growth in the mustache region. He had undergone reconstruction utilizing a single forehead flap by Dr SM Balaji who had published the surgery in the Indian Journal of Dental Research. Taking one of my favorite patients to surgery today. This man has been through hell and back. But his nose is restored and we are doing a little scar touch up and working on the scar on the nose (see the blue marking). I will tell his whole story in a huge video piece as it should be told. The short version is he lost his nose to an operation that should not have been done the way it was done. I did a forehead flap and rib graft and he now looks pretty good! Still working on it a bit today but we are just doing some airway work and scar touch ups.
