The Code says apprentices can request mentor changes. How often does this happen? I imagine the average apprentice wouldn’t consider it unless they were being abused, making it rare, but my new Leopardstar headcanon is that Leopardpaw at one point marched up to her leader and demanded a mentor who’d give her more work to do.
It's a pretty rare thing. It's important to the Clans that apprentices get along well with their mentors, so pairs are chosen with care and communication with the upcoming mentor. Apprentices may wish their mentor was more interested in their rock collection they stowed somewhere special, or that they could have a mentor who was more of a pushover, but it's not usually a big enough problem or case of friction for either of them to complain right to the leader.
Suppose an apprentice's mentor dies suddenly when the apprentice is very close to graduating. Would they be officially assigned a new mentor?
More often than not, yes. Sometimes an apprentice is doing well enough that they can be named early, sometimes they just hang around with the other apprentices and get general training from the rest of the mentors. But usually, they get a "foster-mentor" to run out the clock, just so everyone feels like they're still training properly.

Over time I’ve sorta built up in my head a sorta stoic image of (redux) WindClan, (especially since Rookstar and Deadfoot are so calm) so I’m wondering if the apprentices or kits are ever loud and rambunctious like other clan kits are? Like I know WindClan has the culture of memeish inside humor, and kinda monotone voice, but they’re not emotionless - how do young WindClan cats express themselves? What’s something kits often do that they get in trouble for (but everyone knows they’ll do it again, such as rain kit and sootkit)? If a WindClan apprentice sneaks out, where are they going and what are they doing?

Sorry for the late reply; I saw this, got distracted, and then forgot about it when I got back to my computer.
WindClan kits are expressive for WindClan, but they're much more subdued than the kits of the other Clans. They're at their noisiest when they're nursing and needing the utmost care, where you can expect kits to wail and squeak until their bellies are full. After that, as their ears and eyes open and they begin to understand language, they take after the adults they live with and speak flatly and quietly. They play, of course - that's when they're at their most rambunctious, wrestling and rolling around and chasing and chomping on stray grass. But it's done with less volume than you'd find in RiverClan or ThunderClan.
As they graduate into apprenticeship, their proper personalities develop, but like I've mentioned elsewhere, only a WindClan cat could parse the differences between their Clanmates. It would be very obvious to WindClan that Harepaw is loud and energetic while his sister is sullen and mute, but their neighbors would just see one apprentice talking above the normal volume with little emotion and completely forget the molly next to him. It's a curious thing to witness, unless you're a moor cat and don't struggle to understand the variety. Like with their humor, they don't bother to explain themselves. You just had to be there.
Now, on those last questions of troublemaking in WindClan: sneaking off to the road or Barn, scaring prey and jump-scaring their Clanmates by popping out of the tunnels at random are the most common forms of mischief, but I can say a real trickster can get incredibly creative. There's been an apprentice or two who mimicked snake hisses with perfect precision to scare a kit, or pretended to trip into the Gorge and gotten actually yelled at (the Gorge is taken extremely seriously by adults. No one finds any humor in or around it after enough accidents). I don't think we have time to discuss the amount of young cats that tried to convince a Fang-friend to pretend to chase their Clanmates just to scare them. Not that that ever works. Predators are rarely stupid enough to sour their relationships with the Clan cats for a dumb joke.
