How long after wearing this metal cage should it take for the skin around the balls to toughen up? Some days it feels like it pinches and pulls and others it doesn't bother me.
The most common style of cage is one with a cuff ring around the or is and testicles, with the penis cage attached, leaving a small gap between the bottom of the cage entry and the cuff ring, usually 1/4" to 3/8" (6mm to 10mm). This is called a "trapped ball" design for reasons that should be obvious.
The skin holding your testicles is not necessarily delicate, but it is sensitive.
As your penis tries to erect in the cage, it pushes the cage away from the body, taking that cuff ring with it... and stretching the hell out of your soft, sensitive skin. Most guys experience a burning sensation from this. Anyone who has been to summer camp and who has gotten what we used to call an "Indian burn" will understand why; skin is pliable, but it's not meant to be stretched.
A lot of guys put some lotion around the bottom of the cuff ring so when your body does those grow & shrink things in the night, your skin will have some lubrication to move inside the ring.
Lotion around the ring can also help during the day, as your body moves and pulls things in various directions.
Eventually - weeks to months - your body will adapt to wearing the cage. It will depend on what kind of cage, what style pants, how active you are, etc. This is one of the reasons so many men go through half a dozen different cages until they find one that works for them.
