Lead programmer John Carmack made co-founder of ID Software’s literal head on a stick, the final boss in Doom II. The head can’t be seen unless you’re using idclip (a cheat that allows you to go through walls), but it is in fact what you aim at while fighting the Icon of Sin.

You can even hear the phrase "To win the game, you must kill me, John Romero." backwards as you enter the level.
