And here's the beautiful, elegant problem. The lines were so blurred between what was acting and what was not, that now no-one knows. Not the audience (who were completely taken in by it), not the fans (we spin our own narratives one way and then the other like spiders hoping to trap some evidence), not the actors either sometimes ("I wasn't thinking about the cameras" "I watched Timmy and Armie go at each other"), or even the crew (think of the Director of Photography sobbing on set after the scene where Elio cries "I don't want you to go"). I think Guadanigno knew but didn't think it was important. He expected them to love each other, he's said that in interviews. He expected all of this to happen. And he was absolutely right. Something happened.
