In dnd, resurrecting PCs becomes so easy at higher levels, and though I think that’s a positive, I really like the idea that dying has lasting consequences or triggers events in a PC. Here’s a simple table of interesting but hopefully not game breaking or character destroying ideas. Thoughts? Improvements?

d&d item concept: magical rhyming dictionary that lets you change one word of a spell to a (real) rhyming word of your choice once per day, and the effects of the spell change to match the new word. for example, Vicious Mockery becomes Delicious Mockery, and the target takes 1d4 pie to the face damage
Control Flames becomes Control James. Pretty good if the big bad of your campaign is called James
Hellish Rebuke becomes Hellish Archduke. the grand old duke of hell shows up to fight for you

This is my DM screen.
There's an impending disaster that the players are up against. Something huge and time-sensitive. The only solution (in immediate reach) is the help of a monk. Unfortunately, this monk hasn't been seen in years, and there are rumors that they died or are on a journey around the world. Nobody knows where they are, and there are no physical leads.
Make the players go exploring, but a stressful kind of exploring where they NEED to find something FAST to lead them to the monk. Honestly, this situation is awesome for players who mess around just a bit too much, and can potentially have dire consequences. As always, don't be afraid to kill your PCs if they don't find the monk in time and the disaster occurs.
This situation is much more player-driven. Play around a bit with what you can give them. Maybe at the end of one dungeon there's a scrap of fabric that a player can give to a creature with a superior sense of smell. Or maybe there's a half-finished message scrawled in blood on the wall, signed by the monk. Or maybe the last words of a dungeon boss are something pertaining to the mission. Or maybe there's nothing at all, and it was just a normal, run-of-the-mill dungeon. Make your players frustrated and anxious.
