One of the reasons I feel solid in my reflections on coffee shop etiquette is that I feel just as harsh about my standards for baristas as I do for customers and how they act and I am fair to both sides of the interaction. You're a dick if you don't bus your table when there is a specified place to put your dishes. But - you're also a dick if you are careless about the quality of the drink you put out and I don't feel bad for you if a customer wants to send it back and ask to have it made again. Yes I think you should tip, no I don't think you need to tip $5 to make up for whatever wage a place you don't run is offering. Not remembering to clarify the details of your order and getting the wrong drink is your own fault for not being clear. And if you can't remember if you made the drink using the right milk you should always offer to remake it to be sure not to make someone sick.
