Dottore gives Pantalone the choice and Dottore says goodbye using Pantalone's real name. He sees Pantalone as human so of course he uses his name and Pantalone as someone who sees Dottore as human uses his real name in response.

They see each other for who they truly are and they wanted to show that to each other. Thats why Pantalone gives him the choice.

And when Dottore is talking to Nahida he finally admits what none of the other Dottore's could and sturggled with. That he is human.
