Obviously you know general anesthesia is indicated for this procedure. Any halfway decent doctor would, and you are, despite everything, a halfway decent doctor. But you also know that it’s not necessary.

You know you can skip the analgesic and savor every twitch and squirm when you cut or press in the right spot. You’re pretty good at compensating for the involuntary movement while you’re working. And anyway, a little extra cut never killed— usually didn’t kill anyone.

You also know roughly how much to reduce the sedative dose to get memory blockade without unconsciousness. If you do it right, you get to see the panic rise behind their eyes when they realize they should be asleep by now. Get to see their desperation as they fight the paralytic, trying to tell you something’s wrong. And, if you’re very lucky, their terror when you wink at them and just keep cutting.

If you don’t do it right, and they remember, well. These things happen sometimes. You can read about it. It’s unfortunate, but it’s nobody’s fault. The records will show you used the appropriate dose — you make sure they do. They might think you did it on purpose, and it’s understandable that they’d be upset, but their mind probably fabricated those memories to cope with what was happening. They were sedated, after all.
