Toys 'R' US store employee dies after falling off an 8 foot ladder
DUBLIN, Ohio (AP) — An employee at a Toys "R'' Us store in Dublin has died after falling from an ladder and hitting his head on the floor.
Dublin police Capt. Greg Lattanzi tells The Columbus Dispatch that the man fell from an 8-foot ladder around 7:45 p.m. Wednesday while he was attempting to remove an item from a display. Lattanzi said it appears that he missed a step on the ladder and fell backward, hitting his head.
The victim was pronounced dead at Dublin Methodist Hospital about a half-hour after the incident. His name has not been released because his family is still being notified.
Worker dies in recycling machine 20 feet up

A man working at a Camden glass recycling plant was killed overnight after becoming ensnared 20-feet up in running machinery, authorities said.
Jason Laughlin, spokesman for the Camden County Prosecutor's Office, confirmed the death, but had no further details.
The industrial accident took place in a cullet glass recycling facility on the 2000 block of Mount Ephraim Ave. Glass is crushed and melted into what's known as cullet for reuse.
The lower half of the man's body was reportedly stuck in the machine as it ran.
NY worker dies in shale mine accident

SAUGERTIES, N.Y. — Police say a worker in an upstate shale mine was killed after he became entangled in a piece of equipment.
Police say 30-year-old Stephen Wickham of Greene County was operating an industrial drill late Thursday at the Northeast Solite Corp. in Saugerties (SAW'-gher-teez), boring holes for demolition charges.
They say he got tangled in the shaft of the machine's auger. He suffered severe injuries and died at the scene.
Besides local and state police, the U.S Department of Labor Mine Safety and Health Administration also investigated.
