I think Johnny sees sex as so beneath him and far removed from himself that he also has to force and condition himself to never indulge in it in any way, he can’t think about it or acknowledge that he can even have sex at all. But he still has libido. There’s his own personal reasons for being sex repulsed (*I drew Johnny with the ace flag because it is a popular fandom headcanon, though i think he is sex repulsed and not asexual. sorry) and deeper reasons to why he avoids it, but another reason is because indulging in these things would undermine his insanity. He is not a crazy man doing things with the intention to be a pervert and the weight of his actions being boiled down to just those things would be frustrating.
He doesn’t like thinking about sex or anything like that but that doesn't stop him from having the compulsions for it which probably contributes to some meltdowns and self disgust/hatred.
And necrophilia is even further beneath him, not only is it a sex act but it is also a sex act done with something that has no real sexual appeal to him. Its kind of like when someone with an eating disorder poisons their food so they have no choice but to not eat it, once they’re dead he has no reason to be attracted to the person at all. But I feel like he’d get sort of intrusive thoughts that make him think he is a necrophiliac when he isn't actually.
With his lifestyle sex is so unnatural and forbidden for him that instead of thinking he can achieve it naturally he thinks necrophilia would be more normal or moral. Johnny can't get along with anyone or keep any relationship and he isn't going to take things by force, it's like it must be a reluctant acceptance he knows goes against himself more than it being a developed paraphilia (and again, he isn't ACTUALLY a necrophile from this) so this also would be the only way for him to get anything like that. If you have no choice this would be your last and only option to do it morally. I guess.
These are just unwarranted thoughts though, they’re brief urges that are thought in passing and vary in severity depending how unwell he is. He’d think it’s what “he” wants and then it dwindles out of mind and he forgets about it.
To be clear I don't think he’s capable of having sex in any capacity. With corpses Johnny wouldn’t ever get close enough to it to realize he can't do it, which also just means he can’t disprove he wouldn’t do these things and that perpetuates these thoughts. The worst it'd be is looking at dismembered body parts and thinking things like “Her legs were sexy” or general thoughts and compliments toward appearance while looking at a corpse. These don’t originate with the necrophilia thoughts though, i think regardless Johnny just says these things for pretty corpses since it is objective truths. It’s like mourning someone so you describe all the nice things about them but since he never really knew them the default will have to be to something he can appreciate upon a surface glance.
With more serious relationships cut too soon the compliments would be a bit more personal like thinking “I wish I could keep loving you” which implies he would if he could but the only thing stopping him is the rules he's got to remember to keep himself in check, so take that as you will.
He’s usually too wrapped up in his mind or preoccupied with stuff to realize how crazy these things are to think in retrospect paired with his necrophilia dilemma, if he did he would beat himself up for it but also it's easier for him to excuse his behavior as “im lonely and crazy” since he knows how he feels and it isn't in a perverted way. It's kind of the infamous Johnny trait of separating himself from the majority.
Not really strictly related to the topic of this post but I think Johnny has few morals he follows and they're not from genuine sympathy or empathy. At some point maybe they were made from those things but as he is currently, his morals are more of like a way to not lose himself. His personality fluctuates too severely to be able to make any new rules without contradicting them shortly after or losing the logic as to why to do/not do them. The only ones that have some validity is ones thought up before he went too crazy, it's like a way to remember how he's supposed to be and his past self is his anchor even if now he wouldn't see the reasoning to care about these things. That's not to say he follows them perfectly.
