What are some chronic illnesses that can only occur in a fantasy setting?

Partial transformation - mummy rot is slowly turning you to sand, a near miss from a medusa left you with partially stoned body parts, etc.

Hypnotic suggestions from being mind controlled persist after the controller’s death, causing the victim to occasionally take actions to support the cause of a mind flayer cult that no longer exists.

Repeated demonic possession has left the patient with permanent gaps in their soul’s defenses, causing them to immediately get re-possessed if they go outside a consecrated area.

Post-resurrection trauma as the revived soul remembers an unpleasant afterlife.

Magical healing can get very weird if something is stuck in the wound. It’ll get you back on your feet, but you can get outcomes like “there’s a chunk of wood fused into your chest because the magic couldn’t figure out how to get the arrow out of your chest and just healed it in place,” and this can cause mobility issues or infection vectors down the line.

Healing magic that has a high risk of causing cancer since it causes cells to rapidly duplicate - and that’s basically what cancer is in the first place.

Arcane cancer and mana autoimmune disease. Let’s throw in a disease that gradually replaced the blood with some kind of natural health potion (by forcing the blood to excrete through the pores in the skin), one that gradually overwhelms the nerve structures in the limbs and then the spine and renders the person all but immobile but capable of levitating, and heck how about one that just straight up completely strips the body of magic (used to be an elf, now you’re human, born with the lineage of a dragon, not anymore).

Here's another way magic healing could go wrong! Sometimes, if you're magically healed too much, you lose the ability to heal naturally. Even a small cut or a common cold will kill you if you don't get it magically healed. Even injuries that can't kill you, like a simple scrape, still won't heal on its own.

For a non-healing based one, perfectly regular diseases, but they effect your soul instead of your body. They have all the normal symtoms, drowsiness, cough, stuffy nose, and so on. But physical medicine, healing, or even rest can't help you. Instead, you can actually get healed by self reflection and meditation.
