She asks him what’s wrong, and he seems to hesitate for a moment, but then answers: “Something hurts.”

He’s touching his chest, and Kaori looks concerned and guilty. Then she replies something like: “Oh dear, it’s my bones. My bones are still in there.”

Then she pierces through Tetsuo’s chest with her bare hand, grabs something. Pulls it out. Leaves a bleeding hole. Tetsuo didn’t see it coming and he’s shocked.

Kaori’s holding his literal heart between her now bloodstained cupped hands. It’s still beating, or rather squirming. She smiles sadly, maybe tears up too. Then she says: “It’s beautiful…”

And she begins to fade away. Tetsuo panics and reaches out because his heart is still there and falling from her vanishing hands. Kaori disappears as soon as he catches it. It keeps squirming.

Tetsuo stares at the spot where Kaori was standing just some seconds ago. Then he looks down, and notices that what landed on his hands doesn’t look like his heart anymore. It turned into a fetus.

Tetsuo freaks out and drops it. It phases through the ground, enters a void in which it keeps falling. Turns into a newborn baby while it falls.

By the time it reaches the end of the fall, where Kaneda seems to be waiting, it has taken the form of a small child that strongly resembles Kaori. 

Kaneda catches her. She sleeps in his arms.

He then looks up, as if able to see Tetsuo from there, and once again asks:
