I think he didnt want to say i love you outloud cause he never does say it but its so clear he loves Rose he fell in love with her but yeah he never says cause it would make it more real and he knew in the end he would have to be without her for years and years and saying it outloud would open his hearts letting it all in he didnt want to say it just so that in a few years or even in moments he would to take it away by leaving or dying and she would be left alone without him or vice versa. The Doctor loved Rose immensly but saying it outloud he knew would destroy him and her. So he always showed he loved Rose not in words but in all the million actions they did together and all the million actions he did for her because The Doctor knew actions speak louder than words ever could.
