At what point do you think it is stretched? How long did it take to stretch this far? Did anyone get hurt during this use of the shackle? Will they do it again?
These are the questions I asked when receiving this shackle during an accident investigation. The answers are below.
Stretched too far, Not very long in time, noone was injured (near miss), without a doubt will do it again and have already been done again.
Training, Training, Training, but also management being involved in ensuring these methods of lifting do not continue.
Please help to be safer this year.
Start 2018 off safely and help get those numbers down this year. 5,190 fatalities are too many!!!
