Design is a practice of trial and error, friend. Keep in mind the canon rules. Think about their theme and how it ties into them. Look into mythology related to that theme, or cultures where that theme is prominent. You can gain influence for designs that way. You can also think in terms of their interests, or even their personalities. 

Well, their theme is immortality, so going about it is a little difficult. Especially given only immortality to work off of, finding a symbol that doesn't make immortality blatantly obvious is kind of hard. You could relate it back to other interests and important parts of their character! 

 Infinity is something you could use. Implies a never-ending without making it explicitly obvious that he's immortal. Double endlessness or eternity. 

 Here is an Adinkra symbol relating to the "infinity of man's soul."

Your best bet it that the troll had it, survived it, and suffers some consequence from surviving it. Septicemic plague is one of the blood, and could have done something strange to your fantroll’s bloodtype.Pneumonic plauge is one of the lung, which could give your fantroll a blood-tinged cough from scar tissue, or shortness of breath. I prefer this one because in the time of plague doctors, it was thought that disease infected people through smells. The connection being smell -> inhale -> lungIf they had the Plague of Athens, they might have lost a finger or two. If they had the Plague of Justinian, they might need a new hand.If you’re looking to give your fantroll something your ancestor hat, you best bet might be a congenital disorder. Not all congenital disorders are hereditary, but some are and they’re hella cool.Congenital insenitivity to pain and anhidrosis (CIPA) is “an extremely rare inherited disorder of the nervous system which prevents the sensation of pain, heat, cold, or any real nerve-related sensations (including feeling the need to urinate); however, patients can still feel pressure”.
