In Massachusetts, students in kindergarten through 12th grade are required to be immunized against DTaP/Tdap, polio, MMR, Hepatitis B, and Varicella vaccines for school entry.

Polio: Poliovirus vaccine.
MMR: Measles, mumps, and rubella vaccine.
Hepatitis B: Hepatitis B vaccine.
Varicella: Chickenpox vaccine.
Tdap: A booster dose of Tdap is required for students entering 7th grade.
Meningococcal vaccine: All students entering 11th grade must have received the menACWY (meningococcal vaccine) booster.
Physical Examination: Students are required to submit a recent physical examination (within the last 12 months).
TB assessment/test: Students are required to submit a TB assessment/test.
