Hi Dr. Ferox! Recently my 15 y/o Pug/Pekingese fell ill with what I now know to be pretty obviously Vestibular “Old Dog” Syndrome. I was certain it was going to be her time, but the vet assured me that most likely she would be better in a few days, & she’s improved! I was just curious if this syndrome typically recurs in patients after the first time, if it’s a one & done thing, or anything else of interest! (1/2)

I’m just trying to get as much information from as many (trusted) sources as possible so I know how to help my baby or if there’s anything else I can do to help her be less miserable, lol. (2/2)
Geriatric Vestibular Syndrome (GVS), the old dog one, is a senile change to the vestibular nerve on one side. It’s a fairly benign process, but can have dramatic looking effects and many laypeople mistake it for a stroke. But unlike a stroke, dogs typically recover pretty fast (days) with nursing care.
You can get vestibular syndromes of other sorts, eg associated with brain stem tumors, but these tend not to recover. You can really only tell the difference for certain with a brain scan, but the presence/absence of other neurological defects and lack of recovery could give you some clues.
GVS is caused by a senile change to the vestibular nerve, which runs to the inner ear. This nerve is associated with both hearing and balance. An old dog going deaf is basically par for the course, and many of these dogs do go deaf as well, but it’s the balance component that causes most of the clinical signs. With one vestibular nerve suddenly ceasing to function, the dog’s sense of ‘up’ does not align with the real sense of ‘up, and so you see the typical circling, head tilt, and falling of this syndrome. You also see nystagmus, the eyes drifting slowly in one direction and then flicking back quickly the opposite way. Sometimes there are a few other reflexes which are abnormal too.
Because this syndrome is caused by a change to the nerve and not anything going on in the brain, it is possible for it to occur a second time when the nerve on the other side undergoes the same process. The symptoms just go the other way. If they were circling to the left the first time, they will go to the right this time, and vice versa.
If the nystagmus is ever going up, then it’s not geriatric vestibular syndrome.
Most of treatment for GVS is patiently waiting for the dog to re-learn which way is up, and keep them from feeling too stressed or nauseous in the meantime. Some dogs have an easier time of it than others. Keeping them calm helps reduce their symptoms, and sometimes we keep them in the dark to encourage calmness, but once you can get them eating and walking they usually do well.
While most recover well, many will have subtle hints about their history. They often walk with their legs a little further apart than they used to, called a wide base stance, and may retain a very subtle head tilt.
