There is tartar around the teeth causing inflammation in the gums. 🧚🏻♂️ In one visit, we removed the tartar; removed the decay (cavity) from the front tooth; and made a porcelain crown. 🧚🏻♀️ We even stained the crown with white spots to make it match the other front teeth! 🧚🏻♂️ This is not my usually order for a dentition with multiple teeth with cavities, periodontal inflammation and poor oral hygiene but every case is different because of the patients needs and goals. La mayoría de las infecciones dentales agudas pueden manejarse con éxito a través de intervenciones quirúrgicas, es decir, por drenaje o desbridamiento.
Sigue siendo el fármaco de elección en pacientes alérgicos a betalactámicos por su buena absorción, la baja incidencia de resistencias bacterianas y la alta concentración que alcanza en el tejido óseo. Este antibiótico se muestra muy efectivo frente a anaerobios facultativos y estrictos, incluyendo las cepas productoras de betalactamasas.
Alcanza altas concentraciones alveolares y la actividad bactericida clínicamente se logra con la dosis habitualmente recomendada. Así se describen CMI muy bajas frente a Porphyromonas gingivalis, Prevotella intermedia y Fusobacterium nucleatum. No es activa frente a Actinobacillus actinomycetemcomitans, Eikenella corrodens y Capnocytophaga spp y más de un 25% de los Streptococos del grupo viridans presentan resistencia de alto nivel, no superable con altas dosis de antibiótico. Su propensión a causar colitis asociada a los antibióticos (pseudomembranosa) limita su uso, recomendándose para el tratamiento de infecciones odontogénicas graves o en los casos en que la penicilina ha fracasado. La clindamicina tópica en gel es útil en el tratamiento de la periodontitis en fase aguda y evita los efectos adversos asociados a la administración oral.
Sin embargo, la Asociación Estadounidense del Corazón ha recomendado recientemente recetar clindamicina en lugar de eritromicina como profilaxis de la endocarditis para los pacientes con hipersensibilidad a la penicilina. Como resultado de los crecientes desafíos que causan los géneros bacterianos resistentes a la penicilina, la clindamicina se está convirtiendo en el antibiótico de elección para las infecciones dentales agudas. La posología oral de la clindamicina utilizada en estos estudios fue de 600 mg/d (150 mg 4 veces al día). La clindamicina también ha demostrado su eficacia para tratar las enfermedades periodontales.
Se llevaron a cabo cuatro estudios clínicos en pacientes con enfermedad periodontal refractaria, en la que el tratamiento con limpieza de sarro e higiene oral no fueron suficientes para prevenir la progresión de la enfermedad. En el estudio realizado por Gordon y colaboradores,cada incidencia de periodontitis activa se trató con limpieza de sarro seguida de 600 mg/d de clindamicina durante 7 días. Al cabo de 1 año, las evaluaciones de los pacientes revelaron que los puntos de infección gingival activa habían disminuido del 10.7% al 0.5% después del tratamiento con clindamicina. Otras indicaciones de mejoría obtenida con clindamicina más limpieza de sarro frente a la limpieza del sarro como único tratamiento fueron la reducción del enrojecimiento gingival, del ensanchamiento de las bolsas dentales, del sangrado y de la supuración.
